ABSTRACT

Objective:To explore the application effect of Cornell's note-taking method in pediatric nursing teaching. Methods:A total of 100 nursing students of 2019 who practiced in department of pediatrics were selected as the research objects, and they were randomly divided into a control group and a study group, with 50 students in each group. The control group used traditional teaching, and the research group used Cornell's note-taking method of teaching. After the internship, the teaching effect of the two groups of intern nursing students was evaluated through subject assessment, teaching satisfaction and teaching evaluation. SPSS 22.0 was used for t test and chi-square test. Results:The theoretical knowledge, nursing operation skills and comprehensive ability of the nursing students in the study group were better than those of the control group, and the difference was statistically significant ( P<0.05). The satisfaction degree of nursing students in the control group was 90.00% (45/50), and the satisfaction degree of nursing students in the study group was 100.00% (50/50), with statistically significant differences ( χ2=5.26, P=0.022). More than 88% of interns in the research group believed that Cornell's note-taking method could mobilize learning initiative and enthusiasm, improve learning efficiency and knowledge mastery, improve analysis and problem-solving skills, cultivate assessment and comprehensive judgment skills, and enhance nurse-patient communication skills, thereby improving the overall quality. Conclusion:Cornell's note-taking method can improve the theoretical knowledge, clinical practice skills and comprehensive ability of pediatric nursing students, which is recognized by the interns, and obtains high teaching satisfaction and evaluation.

ABSTRACT

Abstract@#We describe our technique of single-bundle PCL reconstruction using a bony femoral press-fit system. 9 patients underwent PCL reconstruction using our pressfit system. The surgical procedure is described in detail. Post-operatively, 5 patients were available for assessment and review. Four patients gave a final Lysholm score of 92 – 100%. Three patients gave a Hospital for Special Surgery (HSS) score of excellent, one fair and one poor. Two patients gave a reduced Tegner activity score post-operatively while the other three gave a similar score post-operatively. Assessment using KT-1000 revealed four patients with a side-to-side difference of less than 3 mm (average side-to-side difference, 1.87 mm), while one patient exhibited a side-to-side difference of 5.8 mm. We believe that our technique enhances tunnel healing through usage of a bone-plug fixation and provides a cheap alternative for graft fixation on the femoral side in PCL reconstruction.

ABSTRACT

In the present article, we describe a technique of direct intraoperative reduction of congenital atlantoaxial subluxation associated with severe basilar invagination and atlas assimilation. It consists of a wide dissection of the craniovertebral junction, exposing the region between the superior facet of C2 and the occiput (described as a "pseudoarticulation" by some authors). After exposure, a self-rotating small dissector or Kobe is used bilaterally, pushing C2 anteriorly and inferiorly, with reduction of the dislocation, followed by craniovertebral fixation. The technique is safe and eliminates the need for an anterior approach to the odontoid reduction, as well as the need to dissect the C12 joint.

ABSTRACT

PURPOSE: To report a case of cyclic exotropia in a pediatric patient. CASE SUMMARY: There was a pediatric exotropia patient with large variation of prism diopter on alternate cover test at each visits. Not large enough for going through the surgery, she was recommended for the regular check-ups. Sometimes, she was orthophoria and some other times, distinct exotropia. After a few times of follow up, she showed 25-prism diopter exotropia on alternate cover test. The surgery was scheduled and she was admitted after 2 weeks. At the preoperative evaluation, she was orthophoria. She was discharged without having the surgery. To evaluate daily eyeball deviation status, the parents were asked to write a daily note on patient's eye condition. For 3-months, daily strabismus note was thoroughly reviewed and alternate cover test was performed monthly. Patch test was performed twice, and results were 10 and 12 prism diopter exotropia, each. Analyzing the note, a cycle of being the orthophoria and exotropia was noted. With the cycle of 72-hours, she was diagnosed with cyclic exotropia and the surgery was re-scheduled. Ulnilateral lateral rectus recession was performed. Without undercorrection or recurrence, she has been stayed stable over two years of follow up period.

ABSTRACT

The corneomandibular reflex was first described in 1902 by German doctor Von Sölder, who reported the presence of the reflex in patients and in the general population. For him, the corneomandibular reflex represented an exaggeration of the normal response. This was one of the more than 250 reflexes described in the beginning of the 20th century, during the so called "race of the reflexes".

ABSTRACT

The aim of this study was to evaluate the effectiveness of using Mind Maps in teaching anatomy and compare it with traditional based slide. This cross-sectional research was carried out in 2009 on 79 second year medical undergraduate students at Ahvaz Jondishapour University of Medical sciences. Four class lectures on the topics of head and neck osteology were taught. The lectures were slide based designed that were chosen from the chapters in the anatomy textbooks and presented by means of a video projection power point system. The students divided randomly into two groups; one group educated by traditional slides while the other by mind map based designed ones. At the end of the term, a multiple-choice involved forty test was undertaken from two groups. The data was investigated using analysis of variance (ANOVA) and paired T test (p<0.05). The mean scores of females in mind map group were higher than that for traditional one (p<0.04), while there were no significant differences in the mean scores of the men of two groups. The mean scores of females were significantly higher than the males in mind map group (p<0.02) and although the mean scores of females in traditional group was higher it was not significant. This paper has shown the efficacy of using mind maps for teaching gross anatomy as a study aid, even when use has been limited to a single exposure. The study found sex-dependent of learning, suggesting that sex may determine the optimal neural network for designing teaching protocols. Indeed, for females, that have obtained better scores, the use of bilateral neural network seems to facilitate the learning process by mind map teaching of anatomy.

ABSTRACT

The study was conducted on late February 2009, to describe independent variables (socio-demographic characteristics, condom availability, HIV/AIDS information, peer pressure, drug and alcohol use, HIV related knowledge, attitudes \& skills), and to assess any association of these independent variables with the dependent variable (unsafe sex with sex workers) among Myanmar migrant fishermen in Ranong, Thailand. The data was collected using interviewer administrated questionnaire and Focus Group Discussion. The majority of the respondents was in the 26-35 years age group, single, had attained middle school education, Barma, Buddhist, had an income of 4,000-5,000 Baht per month, stayed in Thailand for 1-4 years and never went back home. The mean knowledge score for total respondents was 0.6941. Most of the respondents received HIV/AIDS information and knew where they could get condoms. About 63% of the respondents have had sex with sex workers and 70.7% of them always used a condom when having sex with sex worker. 18.0% of the respondents had casual sexual partners. 40.1% of them always used condom when they had sex with casual partners in the six months previous to the study. In bivariate analysis, The results showed that longer duration of stay in Thailand and earning of more than 5,000 Thai baht, negative attitude towards HIV/AIDS and condom use, and injected narcotic drugs were statistically associated with practice of unsafe sex with sex workers (p-value \<0.005). Receiving HIV/AIDS information, known sources of condom, life skills such as refusing undesired sex, pressure to use drug, to have sex without condom and discussing to use a condom were statically associated with safe sex practice in sex workers (p-value \<0.005). In multivariate, the results suggest that Age, Duration of stay Thailand, Knew where to get condoms, Refusal to have sex without a condom, and Discussing condom use maintained significant association with unsafe sex while controlling for other independent variables. Given the high prevalence of risk behavior, providing health education together with condoms constantly to this group of Myanmar migrant fishermen in Ranong is crucial. Health education should include teaching life skills such as refusal to have sex without a condom, and discuss to use condoms. The 100% condom use program should be promoted among Myanmar migrant fishermen.

ABSTRACT

Migraine and anxiety disorders are frequently co-morbid with balance disorders. Potential mechanisms for migrainous vertigo include sites of action of 5-HT (serotonin) 1B and 1D receptor agonists such as rizatriptan, which attenuate motion sickness in migraineurs. Selective serotonin reuptake inhibitors (SSRIs) are also known to be efficacious in the treatment of vertigo. Relative distribution of the 5-HT receptor subtypes and their functional roles in the vestibular nuclei and inner ear is still unknown. Using 5-HT1A, 1B, AND 1D receptors-specific antibody, we have demonstrated a differential distribution of these receptor subtypes within the rat vestibular nuclei and inner ear. For 5-HT receptor subtypes expression in the vestibular and auditory periphery, most ganglion cells in the vestibular ganglion showed immunoreactivity for 5-HT1A, 5-HT1B and 5-HT1D receptors. In addition, 5-HT1B and 1D receptors immunopositive reactivities were associated with endothelial cells of small blood vessels in the vestibular ganglion and nerve, endothelial cells in both the spiral ligament deep to the spiral prominence and stria vascularis and endothelial cells on blood vessels along the margins of the spiral ganglion. For 5-HT receptor subtypes expression in the vestibular nuclei (VN), the 5-HT1A, 1B and 1D receptors were expressed differentially in the VN. Fine varicose axons in the periventricular plexus showed intense 5-HT1A receptor expression in the medial VN (MVN) and extended into the superior VN (SVN). By contrast, 5-HT1B receptors were not expressed the ventricular plexus axons. Rather, 5-HT1B and 1D receptors immunopositive cell bodies and neuronal processes were dense in rostral MVN, dorsal SVN, lateral VN (LVN) and ventral aspect of nucleus prepositus hypoglossi (NPH). In the present study, inner ear and vestibular nuclei showed distinct distributions of 5- HT1A, 1B and 1D receptors expressions that are parallel to their distribution in peripheral and central nociceptive pathways. These differentially distributed 5-HT receptor subtypes are potential targets to explain the efficacy of SSRIs and triptans in treating migraine and migrainous vertigo.

ABSTRACT

OBJECTIVE: The main complaints of users in existing electronic medical record systems are the complexity of recording system and long recording time. The records of patient moving and the diagnostic test performed outside emergency room are important. But emergency nurses can't record such information exactly and completely because of the highest priorities of emergency treatment and its record in overcrowded emergency room. METHODS: Emergency electronic medical record team of Asan Medical Center developed the automatic recording system of patient moving and diagnostic test information in 2006. The effectiveness of this system was evaluated by comparing the number of records in manual and automatic documentations before and after the system application. RESULTS: The numbers of total records per patient-order were not changed statistically, but the number of manual patient moving and diagnostic test records significantly reduced from 0.50+/-0.69 to 0.31+/-0.47 (p < 0.01). The number of automatic patient moving and diagnostic test records per patient-order was 1.98+/-0.99. CONCLUSION: This automatic recording system can reduce indirect nursing time and give more exact and complete information to emergency medical personnel. The effort to simplify documentation in electronic medical record systems is important, but 'automatic recording' strategy is also needed.

ABSTRACT

This article has been written about an extremely successful case which is interesting and instructive both from the point of view of the methods used and also from the way in which it was detected. In the case, a man dressed in official postman's uniform with a bunch of letters in his hand came to the house of the plaintiff and made him sign some documents as saying these are official directives from the Bailiff's Office. 5 months after this date the plaintiff was informed that proceedings were being taken against him on the part of the Bailiff's Court on account of two promissory notes.

ABSTRACT

OBJECTIVE: HL7(Health Level 7) develops standards for the representation of clinical documents like discharge and consultation notes. The goal of the present study is to develop XML(eXtensible Markup Language)-based communication standard for discharge note. METHODS: This paper presents the use of XML for electronic communication in a document-based EMR, first, as a format for the exchange of structured message, and second, as a comprehensible way to represent patient document. A retrospective analysis of 1165 discharge notes, from the department Seoul National University Hospital, were extracted by querying OCS(Order Communication System) and taking every discharge note of main disease issued over one year period (2003.01.01~2003.12.31). RESULTS: An XML-based prototype for discharge note has been put into place representing the required "section" and "specific instance". In addition, a subset of the CDA(Clinical Document Architecture) Level One details has been described and integrated. CONCLUSION: Through the introduction of definitions for sections and specific instances, progress in the development of CDA Level Two and Three might be realized. An XML-based prototype was implemented, allowing a special view on XML data to generate this document type.
